Information and Support Officer (Physical Disabilities)

Hours: 26 hours per week

Salary: pro rata £27,459 + 3% pension 

Closing date: 11th November 2021

Location: Flexible across the Borough with a central Bromley office base

 We are looking for someone who will provide relevant and accurate information, support, guidance and signposting including both one-to-one and group support for people with complex issues. Support will be flexibly based operating out of a central Bromley office but delivered at various, physically accessible locations, including support sessions in Mottingham and Orpington. 

Main duties and responsibilities:

  • Provide information, support and advocacy on a range of disability related issues for people with complex cases
  • Provide emotional support through telephone, face to face support and home visits where necessary
  • Maintain an accurate record of all enquiries on our comprehensive database and provide reports as required

Sitting Service Coordinator

Hours: 37.5 hours

Salary: £28,000

Closing date: Friday 5th November 2021

Location: Community House, Bromley

We are looking for someone who can lead on the co-ordination of the CQC Registered Sitting Service to facilitate safe and timely discharges and prevent or avoid re-admission to hospital when it is safer to receive care and support in a person’s own home. 

Main duties and responsibilities:

  • Receive, prioritise, cross reference all referrals received for the Sitting Service checking against eligibility criteria for the service and allowing flexibility when appropriate.
  • Gather further information to help with prioritising risk levels and liaise with referring health professionals in hospitals, Transfer of Care Bureau, Community Health Services and Social Services.
  • Promote and grow the service within the PRUH and seek out opportunities to present the services to staff at the hospital.
  • Continuously review the running of the service with a dynamic approach to how it is best ran.

Post Covid Lifestyle Support Worker

Hours: 37.5 hours per week

Salary: £24,947

Closing Date: 24th October 2021

Location: Community House, Bromley

We are looking for a Post Covid Lifestyle Support Worker, who is comfortable working as part of a team with the Bromley Well Lifestyle Coordinator, Lifestyle Support workers & Community Engagement worker. You will be organising and conducting health & wellbeing workshops while taking responsibility for the day-to-day operations of the Bromley Well Long Term Health Conditions programme for Bromley residents who are living with long covid.

We are looking for someone who can:

  • Help with recruitment, coaching and developing volunteer peer health mentors;
  • Take a proactive approach to engage with post covid adults in hard to reach areas of Bromley post diagnosis;

  • Work closely with colleagues on the new Post Covid pathway to ensure clients are well supported to access community support.

  • Support clients to manage their own health conditions.
