Health and Wellbeing Coordinator

Hours: 37.5 per week

Salary: £30,500 per annum

Location: Based at Age UK Bromley and Greenwich, Community House, South Street, Bromley BR1 1RH

We're looking for someone to coordinate and deliver practical health and wellbeing support to adults living with long-term conditions, helping them build confidence, skills, and independence.

This is a varied and hands-on role suited to someone organised, empathetic, and confident working with people who have complex needs, as well as professionals across health and community services.

We’re looking for a motivated Health & Wellbeing Facilitator to join Bromley Well's Long Term Health Conditions team. This is a hands-on role where you’ll support residents living with long-term conditions to build the knowledge, skills, and motivation they need to manage their health more independently.
